Welcome to AFMC. Every fertility journey is different. A helpful first step is choosing a fertility center where you feel comfortable asking questions. To contact AFMC, use the Contact Us page, call (909) 777-8888, or email info@afmc.com.
After you contact us, a member of the AFMC Care Team can explain appointment options and next steps. Once an appointment is scheduled, you can complete the requested forms electronically before your visit.
Initial consultation: An initial consultation is an opportunity to discuss your goals, health history, and questions with the AFMC Care Team. The team can explain which evaluations may be considered and where to find more information about available procedures and testing. If you live outside California or far from the center, contact AFMC to ask which steps may be completed before an in-person visit.
Fertility treatment plan: If you already have a treatment in mind, discuss it during your consultation. Recommendations depend on your individual health information, evaluation results, and fertility goals. The care team can explain available options and the next steps that may apply to you.
Who can I bring to my appointments?
You may bring a friend, family member, partner, or another person you trust. Before the appointment, ask the care team whether any visitor requirements apply.
Where exactly is AFMC located?
AFMC is located at 1370 Valley Vista Drive, Suite 135, Diamond Bar, CA 91765. Parking is available on the north and south sides of the building. You may enter through the northwest or southeast entrance. AFMC is the first office on the left.
What do I bring to my first consultation?
What you need to bring may vary by appointment. After booking, ask the AFMC Care Team to confirm any forms, records, identification, insurance information, or other materials required for your visit.
How do I know what fertility treatment to ask for?
You do not need to select a treatment before your first consultation. The care team can review your goals and available evaluation information with you, then discuss possible next steps.
